Job summary

BAYADA Home Health Care is seeking an experienced Registered Nurse (RN) for full-time home health visits in Forsyth County, NC. You will service adult and geriatric patients in their homes, following care plans and documenting on a state-of-the-art tablet.

Requirements include an active NC RN license and at least one year of clinical experience. Prior home health is preferred but not required; travel to cases is necessary.

Qualifications

• Active North Carolina RN license.
• Minimum 1 year of recent clinical (medical/surgical) experience.
• Home care experience preferred but not required.

Responsibilities

• Follow a designated care plan per patient needs.
• Make home visits to clients in Forsyth County, NC.
• Administer medications, perform wound care, treatments and procedures.
• Monitor client conditions and report changes to supervisor.
• Follow up on doctors' orders and document results.
• Conduct client assessments as needed and coordinate case management.
• Maintain accurate observations and documentation on a tablet and EMR.
• Travel to cases as assigned.

BAYADA Home Health Care is currently seeking an experienced Registered Nurse, RN, for a full time opportunity performing home health visits for our Forsyth County Visits office. This position services adult and geriatric patients in homes throughout Forsyth County in North Carolina.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in North Carolina.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
