Responsibilities:

1. Follow care plans tailored to patient needs.
2. Conduct home visits within designated geographic areas.
3. Administer medications, perform wound care, treatments, and procedures.
4. Monitor patient conditions and report changes to the Clinical or Client Services Manager.
5. Follow doctors' orders accurately and document all interventions.
6. Perform assessments as needed.
7. Manage cases and coordinate care.
8. Document observations, interventions, and evaluations using a state-of-the-art tablet.

Qualifications:

• Current Pennsylvania RN license.
• At least one year of recent clinical experience (medical/surgical).
• Home care experience is preferred but not required.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Proficient computer skills; experience with EMR preferred.
• Ability to travel to assigned cases.
