Responsibilities

• Follow a designated care plan according to patient's needs.
• Make home visits to clients in assigned geographic areas.
• Administer medication, perform wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
• Follow doctors' orders accurately and document properly.
• Perform client assessments as needed.
• Manage cases and coordinate care.
• Document observations, interventions, and evaluations using electronic devices.

Qualifications

• Current RN license in PA.
• At least one year of recent, verifiable clinical experience.
• Prior home care experience preferred but not required.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Proficient computer skills; experience with EMR preferred.
• Ability to travel as needed.
