Key Responsibilities

• Align activities with the vision of Elara Caring’s leadership
• Maintain patient and staff confidentiality per HIPAA
• Assess patients’ physical, psychosocial, and environmental needs
• Manage caseload including assessment, planning, implementation, and evaluation
• Develop and revise care plans based on patient needs
• Communicate significant health findings and changes
• Update care plans and inform team members accordingly

Minimum Requirements

• Current, unrestricted state RN license
• At least 1 year of clinical care experience as a nurse
• Willingness and ability to travel within the coverage area
• Dependable vehicle, valid driver’s license, auto insurance
• Willingness to travel up to 50%
• Associates or Bachelor’s Degree in Nursing preferred
• Previous Home Health or Hospice experience preferred
• Experience with HomeCare HomeBase preferred

You will report to the Clinical Manager or Supervisor.
