• Lake Ozark, Eldon, Versailles
• Columbia

Responsibilities:

• Align activities with Elara Caring’s vision and leadership
• Assess patient and family needs, create and evaluate care plans
• Maintain patient confidentiality per HIPAA
• Evaluate and re-evaluate patient needs during visits
• Communicate significant health findings and changes
• Revise care plans as needed and notify team members

Minimum Requirements:

• Current, unrestricted state RN license
• At least 1 year of clinical care experience as a nurse
• Willingness and ability to travel within coverage area
• Dependable vehicle, valid driver’s license, auto insurance
• Willing to travel up to 50%
• Associates or Bachelor’s Degree in Nursing preferred
• Home Health or Hospice experience preferred
• Experience with HomeCare HomeBase preferred

You will report to the Clinical Manager or Clinical Supervisor.
