Responsibilities:

• Align activities with the vision of Elara Caring’s leadership
• Ensure patient and staff privacy per HIPAA
• Assess patient needs and develop care plans
• Evaluate and re-evaluate patient conditions regularly
• Communicate findings and coordinate with the healthcare team

Minimum Requirements:

• Current, unrestricted state RN license
• At least 1 year of clinical nursing experience
• Willingness and ability to travel within the coverage area
• Dependable vehicle, valid driver’s license, auto insurance
• Associates or Bachelor’s Degree in Nursing preferred
• Home Health or Hospice experience preferred
• Experience with HomeCare HomeBase preferred
