Job Summary

The Registered Nurse in Home and Community Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by the physician or authorized by law. The RN coordinates care plans with other healthcare professionals to ensure continuity of care, exchanging information with physicians, agency personnel, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within scope of practice and infection control standards, following the care plan and coordinating with the care team from admission to discharge.
• Completes clinical nursing assessments as per federal, state, and payer requirements.
• Ensures patient eligibility and medical necessity for services.
• Develops and revises individualized care plans in collaboration with community providers.
• Ensures care plan frequency and duration meet patient needs, revising with physician approval as needed.

Experience Desired

• Minimum of one year RN experience preferred.

License Requirements

• Current RN licensure in the practicing state.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR certification.
