Empath Health is seeking a compassionate Home Health RN to provide in-home nursing care across Pinellas County, including Clearwater, St. Petersburg, Palm Harbor, and Gulfport. You will perform assessments, develop plans of care, and deliver skilled nursing services tailored to patient needs.

Qualifications

• Graduate of an approved nursing program with an active Florida RN license.
• BSN or MSN degree preferred.
• Minimum of 1 year direct patient care experience; home health RN experience preferred.
• Strong assessment, critical thinking, and patient education skills.
• Willingness to participate in weekend and holiday on-call rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes and self-care.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
