Buckeye Home Health Care of Cincinnati is seeking a qualified Home Health RN to deliver expert, compassionate patient care in the homes of patients in Mt. Healthy and surrounding areas.

Your role includes leading patient care with physicians, coordinating LPNs and therapy staff, educating families, and ensuring seamless documentation in our EMR system. OASIS and recertifications will be completed efficiently, supporting high-quality outcomes.

Qualifications

• Active Ohio RN license is required.
• Current CPR certification is required.
• Valid auto insurance is required.
• Previous home health experience preferred.
• Strong communication, organization and EMR skills.
• Compassionate, patient-centered approach.

Responsabilités

• Lead patient care: manage medications, adjust care plans, and prevent rehospitalizations.
• Coordinate the team: guide LPNs, aides, and therapy staff.
• Educate families on care processes and disease management.
• Complete OASIS, recertifications, discharges, and ROCs in the EMR.
• Participate in orientation, staff meetings, and CEU opportunities.

What You'll Do

As a Home Health RN, you will make a direct difference in patients' lives by delivering exceptional care where they are most comfortable - at home.

• Lead Patient Care: Work directly with physicians to manage medication needs, adjust care plans, and help prevent re-hospitalizations.
• Coordinate the Team: Manage and guide LPNs, aides, and therapy staff to deliver holistic care tailored to each patient.
