Job summary

BAYADA Home Health Care is seeking an experienced Registered Nurse (RN) for part-time or full-time opportunities, to perform home health visits for our Triad Senior Living office. Responsibilities include following care plans, administering medications, wound care, and coordinating case management across Forsyth and Guilford counties in North Carolina.

Ideal candidates hold a NC RN license with at least 1 year of clinical experience, and have strong interpersonal and time-management skills.

Qualifications

• A current North Carolina RN license is required.
• Minimum of one year of recent clinical (medical/surgical) experience.
• Prior home health or senior living community experience is preferred.
• Graduation from an accredited Nursing program; ability to travel.

Responsibilities

• Follow a designated care plan in accordance with patient needs.
• Perform home visits to clients in assigned geographic territories.
• Administer medications, wound care, and other treatments.
• Monitor patient conditions and report changes to supervisor.
• Document orders and observations; coordinate case management.

This position services adult and geriatric patients in senior living communities throughout Forsyth and Guilford counties in North Carolina.

Registered Nurse (RN) Responsibilities

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN)

• A current license as a Registered Nurse in North Carolina.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home health or senior living community experience preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
