Job summary

Compass Park in Indiana is seeking an experienced RN Case Manager to join our Home Health team serving Johnson and Bartholomew counties. This role (full-time or part-time) reports to the Director of Nursing and coordinates patient care in the home setting from referral through discharge.

You will review referrals, perform comprehensive assessments including the OASIS within 48 hours, supervise RNs, LPNs, HHAs, and PCAs, work with physicians and families to build coordinated plans, ensure safety

Qualifications

• Graduate of an accredited school of professional nursing
• Knowledge of home health regulatory and reimbursement requirements
• Ability to supervise and direct members of client care team
• Ability to perform paperless documentation electronically
• Licensed driver with automobile insured in accordance with state and agency requirements

Responsibilities

• Reviews client referral information and responds to requests and inquiries as appropriate
• Collaborates with the Director of Nursing regarding eligibility and appropriateness of client for home care services
• Completes comprehensive assessments including OASIS data set within 48 hours of referral
• Performs initial and ongoing client assessments based on agency policy and standards of practice
• Collaborates with physicians, other health care professionals, clients, and families in developing comprehensive, coordinated plan for care
• Manages/supervises a team of RNs, LPNs, HHAs, and PCAs to provide effective and quality home care services
• Promotes personal safety and a safe environment for clients and coworkers and provides care in accordance with state and CHAP regulations
• Maintains confidentiality in all aspects of the job
• Performs other related duties and responsibilities as deemed necessary and completes assigned tasks within established guidelines and time frames
