Job summary

HA Home Care is seeking a skilled Registered Nurse to provide in-home patient care and support. You'll perform Start of Care and Recertification visits, develop individualized care plans, deliver skilled nursing interventions, and coordinate with physicians and the interdisciplinary team.

Qualifications

• Active California RN license is required.
• Graduate of an accredited nursing program.
• Current CPR certification is required.
• Valid CA driver's license and reliable, insured transportation.
• Minimum 1 year of home health, public health, or acute care experience preferred.

Responsibilities

• Complete Start of Care, Recertification, and Discharge visits in compliance with Medicare Conditions of Participation
• Perform comprehensive assessments and develop individualized Plans of Care
• Provide wound care, medication management, IV therapy, and disease-management support
• Communicate with physicians regarding changes in condition and implement orders promptly
• Document all visits and assessments accurately and on time
• Serve as Case Manager for your assigned caseload, coordinating with PT/OT/ST/MSW/HHA
• Supervise Home Health Aides every 14 days
• Participate in on-call rotation as assigned
