Job summary

Responsibilities include assessing patients, developing care plans, administering nursing procedures, documenting OASIS, educating families, and coordinating with physicians and the inter-disciplinary team.

Qualifications

• Active RN license in New Mexico (state licensure).
• Minimum 1 year of clinical nursing experience; home health preferred.
• Proficiency in OASIS documentation and home health regulations.
• Current CPR certification.
• Strong clinical judgment and attention to detail.
• Ability to work independently with reliable transportation.
• Valid driver’s license and auto insurance.

Responsibilities

• Perform initial and ongoing assessments to develop and update personalized care plans.
• Deliver skilled nursing services and specialized procedures per physician orders.
• Complete timely, accurate documentation including OASIS assessments.
• Educate patients and families on safety, medications, and health conditions.
• Communicate with physicians regarding changes in patient condition.
• Supervise and support LPNs and HHAs, including conducting supervisory visits.
• Coordinate care with the interdisciplinary team and participate in team conferences.
• Ensure compliance with all clinical, regulatory, and documentation standards.
