Job summary

Old Colony Elder Services (OCES) is seeking a dedicated Registered Nurse for the Home Care Program to assess and authorize services, coordinating with provider agencies to deliver cost-effective, quality care. You will help establish care plans, determine service needs, and ensure compliance with funding source regulations.

The role requires MA RN licensure, 2 years of clinical/community experience, and strong communication and organizational skills; hybrid work with home visits is available.

Qualifications

• Associate degree in Nursing or related field; bachelor’s degree preferred.
• Massachusetts RN license required; two years clinical/community experience.
• Excellent verbal and written communication; strong organizational and analytical skills; knowledge of elder and disability needs.

Responsibilities

• Determining consumers' eligibility for programs and services according to procedures and guidelines.
• Establishing, reviewing and revising care plans in accordance with funding source regulations and OCES standards.
• Collaborate with care management to determine appropriate service options and cost-effective substitutions.
• Establish service frequency, scope and duration; jointly authorize appropriate packages.
• Maintain records and prepare reports as required.
• Stay updated on OCES policies and funding regulations.
• Act as Lead Assessment Coordinator; participate in interdisciplinary teams.
• Provide consultation to providers and other community agencies.
• Conduct and/or participate in training and workshops as needed.
• Provide consultation to Home Care Program staff.
• Attend to other duties as assigned.

We are seeking a dedicated, caring Registered Nurse for our Home Care Program to be responsible for assessing and/or authorizing services to be delivered by provider agencies with the aim of promoting coordination and cooperation in the formation of cost effective and quality service.

What you’ll do:

• Determining consumers' eligibility for programs and services according to procedures and guidelines
• Establishing, reviewing and revising care plans in accordance with funding source regulations, OCES procedures and standards
• In conjunction with care management team members, determine appropriateness for specific service options; promote cost-effective service substitutions;
• Establish the frequency, scope and duration of services; and jointly authorize appropriate service packages
• Maintaining records and preparing reports as required
• Maintaining knowledge of current OCES policies and standards, and funding source regulations
• Performing role of Lead Assessment Coordinator; participate in interdisciplinary
• Providing consultation to providers and other community agencies
• Conduct and/or participate in training and workshops as needed
• Provide consultation to HC Program staff
• Other duties as assigned.
