Job summary

Emerson Health in Concord, MA is seeking a compassionate Registered Nurse (RN) to join our Home Care team on the Weekend Baylor Program. This weekend, home-based role offers 24 hours per week with a Friday–Sunday schedule from 8:00 AM to 4:30 PM, delivering high-quality, patient-centered care in patients' homes.

We are seeking a compassionate and skilled Registered Nurse (RN) to join our home care team through our WeekendBaylorProgram. This role is ideal for nurses looking to maintain a consistent weekend schedule while delivering high-quality, patient-centered care in the comfort of patients’ homes.

• 24 hours/week - Friday, Saturday, Sunday
• 8:00 AM – 4:30 PM

About the Role:

The Home Care Registered Nurse (RN) is responsible for providing skilled nursing services to patients in their homes. This role includes conducting patient assessments, developing individualized care plans, educating patients and families, and coordinating care to achieve optimal health outcomes.

The RN works collaboratively with interdisciplinary team members to ensure safe, effective, and patient-centered care while maintaining compliance with regulatory and professional standards.

Minimum Qualifications:

Education:

• Graduate of an accredited professional nursing program required
• Bachelor of Science in Nursing (BSN) preferred

Experience:

• Minimum two (2) years of medical/surgical nursing experience preferred
• Experience or knowledge in community health or home health nursing preferred

Licensure and Certifications:

• Current Registered Nurse (RN) license in Massachusetts required
