Emerson Health in Concord, MA is seeking a compassionate Weekend Baylor Program Registered Nurse (RN) for Home Care. This part-time role covers Friday through Sunday, 8:00 AM to 4:30 PM, delivering skilled nursing care in patients' homes and coordinating with an interdisciplinary team.

Weekend Baylor Program, Registered Nurse (RN), Home Care, Emerson Health, Concord, MA

We are seeking a compassionate and skilled Registered Nurse (RN) to join our home care team through our WeekendBaylorProgram. This role is ideal for nurses looking to maintain a consistent weekend schedule while delivering high-quality, patient-centered care in the comfort of patients’ homes.

• 24 hours/week - Friday, Saturday, Sunday
• 8:00 AM – 4:30 PM

About the Role:

The Home Care Registered Nurse (RN) is responsible for providing skilled nursing services to patients in their homes. This role includes conducting patient assessments, developing individualized care plans, educating patients and families, and coordinating care to achieve optimal health outcomes.

The RN works collaboratively with interdisciplinary team members to ensure safe, effective, and patient-centered care while maintaining compliance with regulatory and professional standards.

Key Responsibilities:

• Perform comprehensive patient assessments in the home setting
• Develop and implement individualized care plans and goals
• Provide skilled nursing care and treatments according to physician orders
• Educate patients and families on disease management, medications, and care plans
• Monitor patient progress and adjust care plans as needed
• Document care accurately in the electronic medical record
• Collaborate with physicians, therapists, and care team members to coordinate care
• Maintain patient confidentiality and adhere to professional and regulatory standards

Minimum Qualifications:

Education:

• Graduate of an accredited professional nursing program required
• Bachelor of Science in Nursing (BSN) preferred

Experience:

• Minimum two (2) years of medical/surgical nursing experience preferred
• Experience or knowledge in community health or home health nursing preferred

Licensure and Certifications:

• Current Registered Nurse (RN) license in Massachusetts required
