Job summary

Bond Health in New York operates residential group homes and is seeking a part-time Registered Nurse to evaluate, coordinate, and monitor residents' health needs with a focus on safety and regulatory compliance.

The RN will collaborate with physicians, families, and care teams, develop and update individualized care plans, oversee medication administration, document progress, and participate in quality improvement activities to ensure high-quality nursing care.

Qualifications

  • Current NYSRN license (unrestricted).
  • Current CPR certification.
  • Experience with intellectual and developmental disabilities is preferred but not required.
  • Excellent written, verbal, and interpersonal communication skills.
  • Ability to manage multiple priorities and work collaboratively in a team.
  • Valid NYS or Tri-State driver’s license with good driving record is preferred.

Responsibilities

  • Coordinate medical care and monitor health status of assigned individuals.
  • Assess, document, and follow up on changes in physical condition and overall wellness.
  • Communicate with physicians, families, interdisciplinary team members, and other healthcare providers.
  • Monitor hospitalizations, ER visits, injuries, illnesses, and follow-up care.
  • Develop, update, and maintain individualized nursing care plans and protocols.
  • Oversee medication administration and ensure compliance with medication management procedures.
  • Review medical records, MARs, treatment logs, and related documentation.
  • Monitor dietary and nutritional needs and provide guidance to support staff.
  • Complete nursing assessments, progress notes, and required clinical documentation.
  • Maintain infection control standards and ensure medical supplies are available.
  • Participate in quality improvement activities, meetings, and required training.
  • Perform other duties as assigned.

Position Overview

Bond Health is currently hiring a Registered Nurse (RN) for a part-time opportunity.

The RN will evaluate, coordinate, and monitor the health and safety needs of individuals living in a residential group home, ensuring high-quality nursing care while maintaining compliance with all applicable regulations and standards.

Key Responsibilities
  • Coordinate medical care and monitor the health status of assigned individuals.
  • Assess, document, and follow up on changes in physical condition and overall wellness.
  • Communicate with physicians, families, interdisciplinary team members, and other healthcare providers.
  • Monitor hospitalizations, emergency room visits, injuries, illnesses, and follow-up care.
  • Attend medical appointments when needed to advocate for individuals served.
  • Develop, update, and maintain individualized nursing care plans and protocols.
  • Oversee medication administration and ensure compliance with medication management procedures.
  • Review medical records, medication administration records (MARs), treatment logs, and related documentation.
  • Monitor dietary and nutritional needs and provide guidance to support staff.
  • Complete nursing assessments, progress notes, and required clinical documentation.
  • Maintain infection control standards and ensure medical supplies are available.
  • Participate in quality improvement activities, meetings, and required training.
  • Perform other duties as assigned.
