The Registered Nurse in Home Health provides and coordinates nursing care for patients in their homes, following physician prescriptions and complying with all applicable laws, regulations, and agency policies. The RN collaborates with healthcare professionals, patients, families, and community resources to ensure continuity of care, all within a Point of Care setting.

Key Responsibilities include:

• Providing and directing nursing care to homebound patients as prescribed by physicians, ensuring compliance with laws and policies.
• Coordinating the patient's plan of care with healthcare team members and maintaining continuity through effective communication.
• Conducting initial and comprehensive nursing evaluations, developing care plans within state guidelines, and documenting accurately and timely in collaboration with physicians.
• Performing assessments at designated times and developing appropriate care plans.
• Verifying patients meet home health eligibility and medical necessity criteria as defined by payers.

Qualifications

License Requirements

• Current RN license in Oklahoma
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to reliable transportation
