We are hiring a Full-Time RN for home health visits and admissions in Oklahoma City! $5K Sign-On Bonus!

The Registered Nurse in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies. The RN coordinates the total care plan with other healthcare professionals and ensures continuity of care by communicating with physicians, agency staff, patients, families, and community resources—all within a Point of Care setting.

Responsibilities

• Provide and direct nursing care to patients in their homes, following physician prescriptions and compliance standards.
• Coordinate the care plan with healthcare professionals and ensure continuity of care through effective communication.
• Conduct initial and comprehensive nursing evaluations, develop care plans within state guidelines, and document accurately and timely in collaboration with physicians.
• Perform patient assessments at designated times and develop appropriate care strategies.
• Ensure patients meet home health eligibility and medical necessity criteria as defined by payers.

Education and Experience

License Requirements:

• Current RN license in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to reliable transportation or public transit
