We are hiring a Full-Time Registered Nurse (RN) for Home Health Visits at Apex Home Health, part of LHC Group. The RN provides and directs nursing care to patients in their homes, in accordance with physician prescriptions, laws, regulations, and agency policies. The role involves coordinating care plans with healthcare professionals, ensuring continuity of care, and maintaining detailed documentation within a Point of Care setting.
