Summary

We are hiring a Full-Time RN for Home Health Visits!

The Registered Nurse in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and agency policies. The RN coordinates the total plan of care with other healthcare professionals involved, ensuring continuity of care by sharing information with physicians, agency staff, patients, families, and community resources. All services are delivered within a Point of Care setting.

Responsibilities

• Provide and direct nursing care to patients in their homes, in accordance with physician prescriptions, laws, regulations, and agency policies.
• Coordinate the care plan with healthcare professionals and ensure continuity of care through effective communication with physicians, agency staff, patients, families, and community resources within a Point of Care setting.
• Conduct initial and/or comprehensive nursing evaluations, determine primary care focus, develop the plan of care within state guidelines, and document accurately and timely in collaboration with physicians.
• Perform patient assessments at designated times and develop appropriate care plans.
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by payers.

Education and Experience

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid Driver’s License, vehicle insurance, and access to a reliable vehicle or public transportation
