Responsibilities of a Hospice RN/ RN Case Manager
  1. Report directly to the Administrator, Executive Director, or Patient Care Manager and maintain primary management of each patient.
  2. Serve as the primary liaison between physicians, patients/families, and the hospice team.
  3. Provide direct patient care and supportive care to families, managing a reasonable caseload with compassion and trust.
  4. Educate and empower caregivers on patient care.
  5. Collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to ensure comfort, peace, and dignity.
  6. Assess patient and family needs, and develop and implement care plans.
  7. Maintain accurate clinical records for all patients.
  8. Participate in quality assessment and performance improvement programs.
Qualifications
  • Compassionate, kind, and empathetic nature towards patients and families.
  • Comfortable providing care in patients' homes.
  • Ability to remain flexible and composed during crises.
  • RN license in the applicable state or eligibility to obtain one.
  • At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology fields preferred.
  • Additional experience in hospital settings such as Med Surg, ER, ICU, or critical care is advantageous.
  • Valid driver’s license and insurance.
  • Current CPR certification.
