Responsibilities of a Hospice RN/ RN Case Manager
  • Report directly to the Administrator, Executive Director, or Patient Care Manager and maintain primary control and management of each patient.
  • Serve as the primary liaison between the physician, patient/family, and hospice team.
  • Provide direct patient care to hospice patients and support to their families, managing a reasonable caseload while building trusting, compassionate relationships.
  • Educate and empower caregivers on caring for their loved ones.
  • Collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to deliver comfort, peace, and dignity.
  • Assess patient and family needs and assist in developing and implementing the plan of care.
  • Maintain all clinical records accurately.
  • Participate in quality assessment and performance improvement programs.
Candidate Profile

Requirements for Hospice Registered Nurses:

  • Compassionate, kind, and empathetic towards patients and families.
  • Comfortable providing care in the patient's home environment.
  • Flexible and able to maintain composure during crises.
  • RN license in the state of employment or eligibility to obtain one.
  • At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology is preferred.
  • Additional relevant experience such as hospital staff RN, Med Surg, PACU, ER, critical care, or wound care is advantageous.
  • Valid driver’s license and auto insurance.
  • Current CPR certification.
