Job summary

Professional Healthcare Resources is seeking a Licensed Practical Nurse (LPN) to join our team in the Washington, DC area. You will supplement nursing care for home health and hospice patients under the direction of an RN Field Case Manager.

Responsibilities include carrying out treatments, maintaining records, and reporting status to Clinical and Case Managers. One year of clinical experience is required and home health experience is preferred for this role.

Qualifications

• Must be graduate of a practical nursing program and licensed in state of operation.
• Minimum of one year of clinical experience required.
• Home health or hospice experience strongly preferred.

The LPN will supplement nursing care needs of the home health care and/or hospice patient according to the developed plan of care and under the direction of an RN Field Case Manager.

Requirements

• Must be graduate of a school of practical nursing that meets requirements of state of licensure, and licensed in state(s) of operation
• Must possess sound knowledge of nursing principles, procedures, and standards of practice as determined by state and federal regulations
• A minimum of one year of clinical experience required
• Experience in community or home health strongly preferred
