Registered Nurse (RN) - Emergency Department Case Manager
Within the Emergency Department to proactively identify potential patient admissions and patients with needs for discharge. Works with Bed Center staff to identify inappropriate patient status issues (inpatient versus observation).
Resolves issues of incorrect status through discussions with ED and Admitting Physicians by obtaining additional information to support admission order or determine alternative level of care. Coordinates discharges home from the ED and facilitate transfers to external placement facilities. Maintain expert knowledge of Interqual Level of Care criteria
In addition to the above job responsibilities, other duties may be assigned.
MINIMUM REQUIREMENTS
Education: Graduate of accredited nursing program, Bachelor of Science in Nursing required within 5 years of hire.
Experience: 3 years relevant experience
Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. Certification in Care Management within 3 years of hire.
PHYSICAL DEMANDS
Job requires standing for prolonged periods, frequently traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
