Job summary

onehomehealth is seeking a Part-Time Registered Nurse for Downtown Chicago, IL. The role focuses on delivering high-quality home health care, conducting initial visits, and ongoing patient assessments.

The RN will document Oasis and collaborate with HHA staff and physicians to ensure compliant, patient-centered plans of care. The position emphasizes flexibility, strong teamwork, and adherence to Medicare requirements for safe, effective care.

Qualifications

• Active RN license required.
• Experience in home health nursing preferred.
• Knowledge of Medicare regulations.
• Ability to educate patients and caregivers.

Responsibilities

• Observe, assess, and document patient symptoms and progress.
• Perform initial home care visit and re-evaluate patient needs.
• Demonstrate teamwork with staff.
• Document Oasis accurately.
• Supervise HHA field staff as indicated.
• Follow Medicare regulations.
• Notify physician of patient condition changes.
• Coordinate care with other disciplines.
• Complete paperwork timely and efficiently.
• Educate patients and caregivers on disease processes, medications, plan of care, and treatment plans.
• Provide quality home care and wound care as needed, including IV line care.

• Supervise HHA field staff as indicated
