Job summary

One Home Health LLC in the Chicago Northside Suburbs is seeking a Part-Time Registered Nurse to join our home health team. The RN will perform initial home visits, assess and document patient conditions, and coordinate care with physicians and caregivers.

You will ensure Medicare compliance, educate patients and caregivers, supervise HHA staff as indicated, and deliver wound care and IV care as needed. Flexible schedule and strong work-life balance are highlighted benefits.

Qualifications

• Observation, assessment, and documentation of patient symptoms and progress.
• Perform initial home care visit and re-evaluate patient needs and progress when needed.
• Supervise HHA field staff as indicated.
• Follow Medicare regulations.
• Educate patients and caregivers on disease process, medications, plan of care, and treatment plans according to the policy and procedure manual.
• Provide wound care and assessment; IV line care and maintenance.
