Job summary

South Health and Rehabilitation is seeking a skilled Registered Nurse (RN) to join our long-term care and rehabilitation team. You will provide direct nursing care to residents, support rehabilitation plans, and collaborate with physicians and therapists to maintain high standards of care.

The ideal candidate holds a valid RN license, strong geriatric and rehab care knowledge, excellent communication, and a compassionate, patient-focused approach.

• Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
• Provide education and support to residents and their families regarding care plans and health conditions.
• Maintain accurate and timely documentation in compliance with state and federal regulations.
• Foster a safe, comfortable, and therapeutic environment for residents.
• Supervise and mentor nursing assistants to ensure quality care delivery.
