Shift: Part time - 20 hours per week - weekday evenings from 4 PM to 9 PM, and daytime hours from 7 AM to 5 PM on weekends

Hybrid of days on weekends and evenings during the week.

Summary:

Registered nurses in this role function independently and as a team to receive incoming calls from the after-hours nurse advice lines, referring providers, internal providers, and staff. High customer service orientation with extensive knowledge of clinical systems and EMRs to support clinical integration. The clinical expertise of these nurses will allow for efficient assimilation of information to determine patient acuity, care path, and necessary communication between referring providers, ED Attending physicians, and clinical staff. The nurse will facilitate communication before, during, and after patient visits in the Emergency Department to maintain continuity of care. Communication regarding continuum of care is essential for quality patient outcomes and clinical integration. This nurse will be a point of contact for external providers, embodying high tech and high touch.

Responsibilities:

  • Handle all incoming calls from referring providers, internal or external providers, or families accessing the nurse advice line. Document intake information per guidelines. Monitor referral process for the ED at Legacy and Dallas. Verify bed request orders and notify PCPs of patient admission status. Communicate continuum of care to community providers from the nurse advice line.
  • Ensure continuity of care between ER physicians and referring/primary care physicians, including follow-up after discharge to evaluate patient improvement, share test results, and coordinate follow-up care. Communicate discharge/admission information.
  • Maintain ongoing contact with patients and families during ED visits to improve communication.
  • Investigate complaints from primary care and referring physicians in cooperation with hospital leadership. Coordinate ED service issues with physicians.
  • Engage in professional development and maintain knowledge of hospital, department, and community.
  • May travel to physicians' offices in North Texas.
  • Perform other duties and special projects as required.

How You’ll Be Successful

Work Experience:

  • At least 5 years of clinical experience required.
  • At least 3 years of emergency triage with pediatrics and/or adults preferred.
  • At least 5 years of telephone triage experience preferred.
  • Other critical care nursing experience considered.

Education:

  • Bachelor's degree or equivalent experience preferred; Associate’s degree with pediatric emergency experience considered.

Licenses and Certifications:

  • Current Texas nursing license required.
  • Basic Life Support (BLS) required.
  • PALS within six months of hire required.
  • CEN, CPEN, bilingual preferred.
