Job summary

Comfort Keepers is seeking an experienced Registered Nurse (RN) Case Manager to join our team. This role conducts client assessments, develops and manages care plans, and performs supervisory visits for seniors and veterans in Oklahoma.

Under supervision of the General Manager and Director of Clinical Services, you will support caregiver training, ensure quality client service, and travel to client homes as needed. Flexible part-time, PRN or contract scheduling is available.

Qualifications

• Active and unrestricted RN license.
• Minimum of 3 years in Home Health, Hospice, Rehabilitation, Home Care, or PDN.
• Valid driver's license and reliable transportation; ability to travel.
• Strong computer and electronic medical records skills.
• Excellent communication and interpersonal abilities; compassionate.

Responsibilities

• Conduct intake calls and in-home assessments.
• Develop, implement, and document individualized care plans.
• Perform supervisory visits and coordinate caregiver training.
