Job summary

Paris Regional Health seeks an experienced Registered Nurse (RN) Case Manager to coordinate patient care, discharge planning, and resource referrals in Paris, Texas. You will work with nursing staff and the multidisciplinary team to assess care needs and ensure quality outcomes for patients across the continuum of care.

You will apply your specialized plan-of-care interventions, monitor patient progress, and leverage our care-technology tools to improve patient wellbeing.

Qualifications

• Current state RN license is required.
• Associate degree from an accredited nursing school is required.
• BLS certification required within 30 days of hire.
• ACLS and PALS are required within six months of hire.
• One year of RN experience preferred.
• ASLS certification preferred.

Responsibilities

• Consults with nursing staff and multidisciplinary team regularly to evaluate patient status and appropriateness of care.
• Monitors patient and family satisfaction and responds to questions from patients, families, and payors.
• Participates in discharge planning including coordinating transfers and community resources.

How you’ll contribute

You’ll make an impact by utilizing your specialized plan-of-care intervention and serving as a patient-care innovator. You will shape exceptional patient journeys every day and leverage your skills and our cutting-edge technology to directly impact patient wellbeing.

Essential Functions

Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
Monitors patient and family satisfaction. Responds to questions and complaints from patients, family members, and payors regarding care.
Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources. Provides discharge education and resource referrals to patients.
Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.

Qualifications and requirements

Applicants should have a current state RN license and possess a associates degree from an accredited nursing school. Additional requirements include:

• Basic Life Support certification is required within 30 days of hire.
• ACLS and PALS are required within six months of hire.
• One year of experience preferred
• ASLS certification preferred
