Job summary

Adaptive Home Health, LLC is seeking a part-time Registered Nurse (RN) Case Manager to deliver high-quality in-home care in the Detroit-Brighton area. You will manage complex caseloads, document thoroughly, and collaborate with physicians and teams.

This role emphasizes patient outcomes, efficient charting via AI-enabled platforms, and strong relationships with referral sources. Field-based work with flexible scheduling supports work-life balance.

Qualifications

• Home health experience strongly preferred.
• OASIS documentation experience preferred.
• Reliable transportation and valid driver's license.

• Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Direct, high-acuity patient care with focus on wound care.
• Building and managing the plan of care with physicians; supervising LPNs and HHAs.
• Being the face of Adaptive to patients, families, and referral sources.

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

1. Carry a high-quality caseload, with OASIS accuracy that holds up to review.
2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

Requirements

• Active Michigan RN license
• Reliable transportation, valid driver's license, auto insurance
• Home health / OASIS experience strongly preferred
