Job summary

Phoenix Home Care & Hospice in Kansas City, MO is seeking an RN Case Manager to deliver compassionate hospice nursing care to patients and families within the community. You will work with the interdisciplinary team to develop and implement a patient-centered plan of care that guides care decisions and promotes quality of life.

Ideal candidates hold an active Kansas or Compact RN license, hospice experience is preferred (training provided), and a valid driver’s license with auto insurance.

Kansas City, MO & Surrounding Areas Pay Range: $80,000 - $90,000

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life. Our ideal candidate should possess a high level of energy and motivation, coupled with strong managerial, organizational, and interpersonal skills. They must be capable of providing precise and effective guidance and support, even in demanding situations, in order to enhance learning opportunities and foster professional growth.
