Job summary

Northshore Home Health Care is seeking a Registered Nurse Case Manager to deliver comprehensive hospice care, develop personalized Plans of Care, and coordinate treatment strategies that support patients' physical, psychosocial, emotional, and spiritual needs.

Responsibilities include initial eligibility evaluations, reassessing patient needs at least every 15 days, documenting care progress, collaborating with patients, families, physicians, and interdisciplinary teams, and providing education

Qualifications

• Current, unrestricted RN license required in South Carolina.
• CPR certification preferred.
• Reliable transportation is required.
• Over 1 year of clinical hospice or home health experience preferred.
