Job summary

OSF HealthCare is seeking a Registered Nurse Home Care to deliver comprehensive patient care from infancy through geriatrics. The RN Case Manager coordinates inter-disciplinary teams to ensure safe, effective care during each episode and manages visit patterns and outcomes.

• Current licensure by the State Board of Nursing in Illinois or Michigan.
• Current American Heart Association HealthCare Provider BLS is required before start date.
• Maintains a valid driver's license and own transportation.
• Healthcare experience preferred (RN home care) or equivalent.

Responsibilities

• Coordinate inter-disciplinary care for patients (nursing, therapy, social work, home care aide, equipment) to ensure safe and effective episode management.
• Provide patient-centered nursing care across all ages, from infancy to geriatric.
• Manage visit patterns, outcomes, resources and episode progression.

Pay range for this position is $36.59 - $53.17/hour. Actual pay is based on years of licensure. This is an Hourly position.

Overview

POSITION SUMMARY: The Registered Nurse Home Care is responsible for a scope of care that encompasses all ages from infancy through geriatric. The RN demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. Coordinates inter-disciplinary care to patients, which may include nursing, therapy, social work, home care aide, home medical equipment and home infusion services as needed. The RN Case Manager is responsible for decision-making regarding the home health care the patient receives during their episode of care. Provides patient-centered care, using critical thinking skills to ensure positive patient outcomes. The RN Case Manager is responsible for care coordination, appropriate visit pattern utilization, outcome monitoring/management, resource management and episode management.

Qualifications

REQUIRED QUALIFICATIONS:

• Licenses/Certifications: Current licensure by the State Board of Nursing in appropriate state (Illinois or Michigan)
• Current American Heart Association HealthCare Provider BLS is required prior to start date of employment.
• Experience: Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty.
• Maintains a valid driver's license.

PREFERRED QUALIFICATIONS:

• BSN
• Experience as an RN in a healthcare environment
• Electronic Medical Record and Microsoft application basic proficiency
