Blue Ridge Care is seeking a compassionate Registered Nurse (RN) Case Manager to join our hospice team in Frederick/Clarke County. You will provide direct patient care, coordinate personalized care plans, and collaborate with an interdisciplinary team to support body, mind, and spirit.

Ideal candidates hold a Virginia RN license, CPR, and at least two years of nursing experience in hospice or home health. A $10,000 sign-on bonus, competitive pay, shift differentials, PTO, and growth

• CPR certification and at least two years of nursing experience (hospice or home health preferred).
• Strong clinical judgment, communication, and organizational skills.

About the Role

With a strong focus on clinical judgment, communication, and organizational skills, you'll be responsible for:

• Leading and guiding hospice aides to ensure consistent, compassionate care
