Job summary

Blue Ridge Care is seeking a compassionate Registered Nurse (RN) Case Manager to join our hospice team in Frederick/Clarke County. You'll provide direct patient care, coordinate personalized care plans, and support patients and families with clinical judgment and empathy.

You'll collaborate with an interdisciplinary team to ensure holistic care—body, mind, and spirit—while focusing on symptom management, education, and timely documentation.

Qualifications

• Active Virginia RN license is required.
• CPR certification is required.
• At least two years of nursing experience, hospice or home health preferred.

Responsibilities

• Assessing patient and family needs and developing personalized care plans.
• Delivering skilled nursing care and symptom management.
• Administering medications and coordinating care across the healthcare team.
• Educating and supporting families to promote comfort and peace of mind.
• Leading hospice aides to ensure consistent, compassionate care.

About the Role

As an RN Case Manager, you'll provide direct patient care, coordinate individualized care plans, and serve as a trusted presence for patients and families. You'll collaborate with an interdisciplinary team to ensure every patient is cared for - body, mind, and spirit. With a strong focus on clinical judgment, communication, and organizational skills, you'll be responsible for:

What You'll Bring

• Current RN license in Virginia (multi-state or single-state)




• CPR certification and at least two years of nursing experience (hospice or home health preferred)

• Reliable transportation, valid driver's license, and insurance
