Job Summary

Responsible for facilitating patient care along a continuum through effective resource coordination to help patients achieve optimal health, access care, and utilize resources appropriately, considering their resources and right to self-determination. Ensures care is provided at the appropriate level based on medical necessity, assesses transition needs for timely throughput, safe discharge, and readmission prevention. Supports standards for care management including Utilization Management, Transition Management, and Care Coordination, while complying with regulatory standards and providing education to physicians, patients, and caregivers. Completes required competencies within the designated period and performs other duties as assigned.
