Job summary

Faith Hospice is seeking an experienced Registered Nurse (RN) to serve as a Hospice Case Manager. The role is part of an interdisciplinary health care team dedicated to delivering compassionate end-of-life care with dignity.

You will work a Monday–Friday daytime schedule, participate in on-call rotation, and manage a caseload within a 50-mile radius of Grand Rapids to spend more time with patients and families at home or in the community.

Qualifications

• RN licensure in Michigan.
• At least 2 years of med/surg or acute care nursing experience.
• Hospice and/or hospital experience preferred.
• Passion for hospice nursing.

Responsibilities

• Work a Monday – Friday daytime schedule.
• Participate in a back-up on call rotation with other RNs; this equates to the equivalent of one calendar week every 4 months (5pm – 10pm on weeknights and 9am – 9pm on Saturday and Sunday).
• Manage a caseload of hospice patients within an assigned geographic territory to minimize drive time and spend more time with your patients (we cover a 50-mile radius around Grand Rapids).

What Will You Do?
