Job Description

As a case management nurse (RN CM), you will collaborate with the healthcare team to assess, plan, facilitate, and coordinate care for individual patients. Working within and outside of facilities, you will oversee the effective utilization of services and resources. You will communicate with patients, families, other healthcare professionals, and social workers to promote patient safety, quality of care, and cost-effective outcomes, especially for patients requiring long-term or home care after hospitalization.
