Registered Nurse (RN) Care Manager - Maryview Medical Center

Position Overview

The RN Care Manager coordinates patient care to ensure safe, seamless, and timely transitions across the continuum. Using collaborative methods, the RN assesses, plans, implements, and evaluates services addressing health and social determinants affecting wellbeing. Responsibilities include screening, assessment, and promoting appropriate resources for active Case Management.

Key Responsibilities
  • Identify and prioritize patients needing care management, considering biopsychosocial, cultural, spiritual, and financial factors. Collaborate with patients, caregivers, and healthcare teams to optimize outcomes and adjust plans as needed.
  • Document all care activities clearly and promptly in patient records, including transition and discharge planning, involving patients and families to incorporate their goals and preferences.
  • Follow standardized practices for Advance Care Planning, length of stay management, and readmission prevention.
  • Support denial prevention by addressing barriers and participating in interdisciplinary discharge rounds.
  • Assist in removing barriers to timely testing and treatment, ensuring appropriate resource utilization and offering alternatives to acute care.
  • Collaborate with revenue cycle teams to classify patients correctly and ensure timely delivery of regulatory correspondence.
  • Participate in clinical outcome projects and process improvements within the department.
  • Work with peers to meet departmental goals through effective communication and teamwork.
  • Maintain compliance with CMS and Conditions of Participation regarding discharge planning and utilization management.
Qualifications
  • Current BLS certification from the American Heart Association
  • RN license in the state of practice or covered by a compact license
  • Preferred: Certified Case Manager (ACM, CCM, or ANCC Nursing Case Management certification)
  • Required: Bachelor of Science in Nursing; preferred for some locations
  • Minimum of 1 year clinical experience; 3 years in acute care preferred
