Role Overview

Collaborates with the clinical team to identify patients who benefit from care coordination, assessing risk factors, clinical utilization, and preventive services. Ensures continuity of care across inpatient, emergency, and outpatient settings, including discharge planning and follow-up care.

Position Details

• Department: GI
• Status: Full Time
• Shift: Days (7:30am-5:00pm, Monday-Friday)
• No weekends or holidays

Responsibilities

• Lead in ensuring care continuity across care settings, including discharge planning and follow-up.
• Develop relationships with patients, families, social workers, and nurses to support care plans.
• Monitor progress and adjust discharge plans as needed, especially for high-risk patients.
• Educate patients and families on chronic illness management and healthcare system navigation.
• Identify support needs, develop action plans, and assist with resources and services.
• Use IT tools to track patient populations and improve outcomes.
• Participate in clinical outcome measurement and promote population health.
• Ensure safety and compliance with policies and standards.

Qualifications

• Effective July 1, 2024: Bachelor of Science in Nursing (BSN) required for new hires or transfers.
• Current RN license in Florida.
• Current BLS/Healthcare Provider certification.
• Minimum 3 years of clinical experience, with emphasis on the served population.
