Role Purpose

Coordinates healthcare for specific patient populations across the continuum of care, managing patient contact, education, and preparation for procedures in collaboration with the healthcare team.

Responsibilities
  • Assess and evaluate patients' physical, psychosocial, and emotional needs to facilitate patient care.
  • Coordinate delivery of safe, evidence-based patient care.
  • Monitor care data to optimize patient outcomes.
  • Educate patients and families; serve as a resource to the healthcare team.
  • Ensure actions align with business goals.
