In this role, you’ll provide essential, community-based care to adults living with serious mental health and substance use challenges.

You will develop meaningful relationships with clients, support them through real-life challenges, and ensure seamless continuity of care across community environments. You’ll collaborate closely with psychiatrists, therapists, co-occurring disorder specialists, and other behavioral health professionals to deliver truly integrated care.

Why Join Us?

At Easterseals PORT Health, we invest in your growth, your well-being, and your ability to thrive in your career. You’ll join a mission-driven ACT Team that values collaboration, community engagement, and exceptional care delivery.

This full-time role follows a Monday–Friday schedule with rotating on-call responsibilities. Community travel is required.
