Job summary

Easterseals PORT Health is seeking a full-time Registered Nurse (RN) to join our ACT Team, providing community-based behavioral health care for adults with serious mental health and substance use challenges. This role involves in-home and community setting nursing, medication management, and care coordination.

Join a mission-driven team focused on stability, independence, and quality of life, with a Monday–Friday schedule and rotating on-call responsibilities.

Qualifications

• Active, unrestricted RN license in North Carolina.
• Experience in behavioral health or mental health (preferred).
• Comfort working in community-based settings.
• Valid driver’s license, reliable transportation, and vehicle insurance.
• Strong communication, documentation, and relationship-building abilities.

Responsibilities

• Administer and monitor psychotropic medications as prescribed.
• Conduct nursing assessments and track symptoms, side effects, and medical issues.
• Document all care and observations in the EMR.
• Partner with the team psychiatrist to update medication protocols and care plans.
