Job summary

Evergreen Woods in Connecticut is seeking a compassionate caregiver to provide resident care in accordance with the Resident Care Plan and to assist with daily living activities.

You will document care decisions promptly, interact with physicians, pharmacists and medical staff as needed, and support residents with bathing, dressing, eating, toileting, hygiene and mobility. Ongoing training and other duties may be assigned.

Responsibilities

• Keeps timely documentation of written reports and verbal reports ensuring quality care.
• Interacts with the resident's physician, pharmacist and medical personnel as needed.
• Assist residents with activities of daily living including bathing, dressing, eating, toileting, hygiene, and mobility.
• Participates in and attends all required in-service training and education sessions as scheduled.
• Other duties as assigned.
