Job summary

Evergreen-Woods is seeking an Registered Nurse (RN) for a 3pm-11pm shift in a skilled nursing setting. The role emphasizes timely documentation and coordination with physicians, pharmacists, and other medical staff to ensure quality resident care.

The RN will assist residents with daily living activities as outlined in the Resident Care Plan, including bathing, dressing, eating, toileting, hygiene and mobility, and will participate in mandatory in-service trainings and other duties as assigned.

Responsibilities

• Keeps timely documentation of written reports and verbal reports ensuring that quality care is provided.
• Interacts with the resident’s physician, pharmacist and medical personnel as needed.
• Assist residents with activities of daily living as follows in the guidelines as outlined in the Resident Care Plan including bathing, dressing, eating, toileting, hygiene, and mobility.
• Participates in and attend all required in-service training and education sessions.
• Other duties as assigned.
