The Ambulatory Telehealth RN provides comprehensive nursing care remotely via telehealth services. Responsibilities include patient assessment, triage, care coordination, education, population health management, and patient navigation using telehealth technology. The RN collaborates with providers and staff to ensure high-quality, patient-centered care.
Responsibilities
- Engage in population-appropriate communication.
- Understand growth and development milestones.
- Provide clear treatment instructions to patients and families.
- Involve family/guardians in assessment and care.
- Identify physical limitations and intervene when necessary.
- Recognize and respond to behavioral health issues.
- Interpret data and plan care accordingly.
- Address psychological, cultural, and religious needs.
- Identify signs of abuse or neglect.
Patient Assessment & Interventions
- Conduct initial assessments via telehealth, including hospital and ED discharge assessments.
- Use the nursing process to address patient concerns.
- Monitor response to care plans.
- Manage inbound calls from patients, families, and care teams.
- Follow policies for triage and escalation.
- Develop individualized care plans with healthcare providers.
- Recognize abnormal labs/results and collaborate on changes.
- Anticipate patient needs and act proactively.
- Coordinate care and facilitate referrals.
Patient Education
- Educate patients/families on health conditions, treatments, medications, and self-care.
- Guide on managing chronic and preventive care.
- Reassess understanding and re-educate as needed.
- Support understanding of telehealth processes.
- Document educational outcomes.
Documentation & Communication
- Maintain accurate, timely documentation.
- Ensure confidentiality and data security.
- Use MIDAS for safety concerns.
- Communicate test results and follow-up plans.
- Address concerns and support remotely.
- Escalate issues appropriately.
- Collaborate with social workers, dietitians, and other professionals.
- Document collaborative care plans.
- Follow up and use closed-loop communication.
Technology Utilization
- Use Epic, Microsoft Office, and telehealth technology effectively.
- Troubleshoot technical issues.
- Stay updated on new telehealth tools and practices.
Leadership & Professional Development
- Participate in committees and policy development.
- Implement process improvements.
- Promote accountability and standards.
- Engage in orientation and education programs.
- Stay current with research.
- Contribute to staff meetings.
Qualifications
- Associate's Degree in Nursing required; Bachelor's preferred.
- Current RN license in Ohio.
- BLS certification preferred.
- Minimum 2 years nursing experience; telehealth experience preferred.
