Job summary

Northwell Health is seeking a Registered Nurse to deliver age-specific care using the nursing process in a collaborative, leadership-forward role. Based in Sharon, CT, this full-time position emphasizes comprehensive patient assessment across physiologic and psychosocial domains, care planning, and education for patients and families.

The role requires an Associate’s Degree in Nursing, 1–2 years of nursing experience, and a New York State and/or Connecticut RN license with BLS.

Qualifications

• Associate’s Degree in Nursing is required.
• Bachelor’s Degree in Nursing is preferred.
• 1–2 years of professional nursing experience in the related specialty.
• Current license to practice as a Registered Professional Nurse in New York State and/or Connecticut is required.
• BLS certification required; specialized certifications as needed.

Responsibilities

• Assess the patient’s physiologic health status.
• Conduct patient interviews and explain policies and procedures.
• Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status.
• Determine mobility, sensory deficits, prostheses use, and skin condition.
• Assess level of pain and pain management.
• Communicate and document patient’s physiologic health status and plan of care.
• Assess patient’s psychosocial health status.
• Elicit perception of medical/nursing care and expectations.
• Develop criteria for measurement of patient outcomes.
• Formulate the plan of care and establish priorities to achieve expected outcomes.
• Implement the plan of care and maintain constant vigilance over patient care.
• Evaluate care provided for patient outcomes.
• Collaborate with other care team members in planning and carrying out treatment regimens.
• Administer medications correctly and safely.
• Participate in patient and family education.
