Are you a compassionate and experienced Registered Nurse (RN) who thrives in delivering care when it matters most? Mountain Valley Hospice is seeking an Afterhours RN Team Leader to provide essential oversight and support to our dedicated on-call nursing team. This role ensures that patients and families receive top-quality care throughout the evenings, nights, and weekends-exactly when they need it.

Key Responsibilities:

• Clinical Resource and Support: Act as the leader for the afterhours nursing team, providing guidance and support ensuring that team members feel confident in their care delivery.
• Patient Response and Oversight: Address urgent patient needs, admissions, transfers, and other afterhours inquiries as they arise.
• Documentation: Oversee accurate and timely entry of patient information into the Electronic Medical Record (EMR) to maintain comprehensive and up-to-date records.
• Scheduling and Staffing: Collaborate on staffing assignments and scheduling to ensure adequate on-call coverage, optimizing our team’s readiness and responsiveness.
• Direct Patient Care: Step in to provide hands-on nursing care to terminally ill patients if needed, ensuring that our patients receive continuity of care at any hour.

Schedule and On-Call Availability:

• Rotating Shifts Between Two Team Leaders, every other weekend
• Monday through Friday: 5:00 PM - 8:00 AM (On-Call)
• Saturday - Sunday 8:00 AM - 12:00 AM (On-Call)

Qualifications:

• Active Registered Nurse (RN) License
• BLS Certification
• Valid Driver’s License
• Strong leadership skills, clinical expertise, and adaptability for the on-call nature of the role.
